Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (954) 475-9244 during regular business hours.
Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, Visa/Mastercard.
Questions About Your Bill? - If you have a question about your bill, you may contact our Billing department, during regular office hours, at (954) 475-9244.
You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
We recommend that you call your insurance company to verify that we participate with your plan. Regarding insurance plans where we are a participating provider - all co-pays and deductibles are due at the time of service. In the event that your insurance coverage changes to a plan where we are not a participating provider, the balance is your responsibility.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
Various insurance companies require an inter-office referral. Please bring the referral with you if our office has not received it in the days preceding the appointment. If a co-payment is due for your visit, the co-payment is due the day of your appointment.

If you have had any testing (MRI, X-Rays, CT scan and/or an EMG) done, please bring a copy of the report along with the actual study if applicable. Your appointment will be rescheduled if you come to your appointment without the studies. You may pick up these films at the imaging center where the test was performed. Please call the facility 24 hours in advance to have your films or compact discs ready.
Cancellations or Rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
We only renew prescriptions that we initiated. For other medications, please contact the health professional who prescribed them.
We can fill many renewal requests by phone. Please contact our office at least ten (10) business days before you expect to run out of medication. Sometimes, for medical reasons, we will need to see you before issuing the prescription.
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
After-Hours & Emergencies
